We serve all of Capital Health Region, and catchment area (northern Alberta, far north of Canada). We see children from antenatal period to 18 years with any life-limiting or life-threatening condition.

The implementation will see the redesign of both hospital and community based services for children & young people, with an emphasis on providing care closer to home and keeping children out of hospital beds. The model is based on safe, accesible services with the child & family at the centre.
